Car Nicobar, March 23: The patient named Henry was admitted to hospital with complaints of severe pain in the lower abdomen since past one week. Upon examination it was detected that there was a mass in the lower abdomen and right side absent testis. Ultrasound showed a vascular mass of size 8* 10cm in the suprapubic region.
The patient was immediately taken up for surgery on 12th of March and on opening the abdomen it was found to be a suspected seminoma of the right sided testis, which had undergone twisting on itself 2 times (torsion) and vascular in nature.
The seminoma mass was detorted, excised and the patient is said to have recovered uneventfully. The nature of the mass is to be confirmed with histopathology and may require further chemotherapy.
Diagnosis of the condition revealed suspected seminoma of the intra-abdominal right undescended testis with 720 degree torsion for which detorsion and excision of the suspected seminoma mass was done.
Normally when babies are born, both the testis will be descended in to the scrotum. But in some kids it won’t be descended. This condition is called undescended testis. In such cases a wait period of 6 months to descend is observed after which it is unlikely to descend. In such a situation surgery is required to be done at the earliest to bring the testis in to the scrotum. If surgery is not done, the testis will undergo degeneration and damage which might make the patient infertile, conversion in to cancer like seminoma, torsion.
As a precautionary measure, mothers should check their babies’ scrotum for the presence of both the testis and if any problem should seek a surgeons opinion immediately.
The entire procedure was carried out in BJR Hospital, Car Nicobar headed by Dr. Bharath, Surgical Specialist.
